I am sure that people can come up with all kinds of reasons, but the vendors and customers involved like proprietary systems so that unbundling can't happen.  The more open the environment the more likely that you would have to share.

From a vendor standpoint, you get both ends of the line all the time.

From an MSO standpoint, your customers can not get gear from anyone else.

AT&T was not big on Carterphone either.
